Since the 1995 publication of Bad Blood, The Tragedy of the Canadian Tainted BloodScandal much has changed in the Canadian blood collection and distribution system.

Despite this, many of the issues that arose in the 1980s resulting from
contamination of the blood supply with HIV and Hepatitis C (HCV) still
haunt the governments, agencies and consumers of blood products and
donors.

Many of these changes were driven by the $15 million report prepared
under the direction of Justice Horace Krever whose commission
conducted a detailed examination of Canada’s blood system in the
1990s. For example, the Red Cross is now out of the blood system
replaced by Canadian Blood Services and Hema-Quebec, stand-alone
agencies whose only responsibility is collection and distribution of blood
and its products.

The reissuance of an updated version of Bad Blood comes at a time
when an eight-part mini-series, Unspeakable, is being prepared for TV
distribution. The series, based in part on Bad Blood and on the personal
experience of executive producer Robert C. Cooper, who was infected
with HCV through blood products, is expected to air in early 2019. It is
being funded by CBC and Sundance TV.

We took our country’s blood supply for granted
— safe, available, life-saving.

We did so at our peril. We now know that more than a thousand Canadians who had transfusions in the late 1970s and 1980s—hemophiliacs, surgery patients, accident victims, women and their babies in childbirth—were infected with HIV. More than 70 percent have since died of AIDS. Thousands more were infected with preventable hepatitis and have truggled with cirrhosis and even liver cancer.

Hemophiliacs were the proverbial canaries in the coal mine—an early warning sign that there was something terribly wrong with the Canadian blood supply. Yet officials denied there were issues at all, and in fact covered up clear evidence that they had failed to take sometimes simple actions that would have saved countless lives. We cannot allow the agencies that protect our health care system to look the other way or put budgets before patients.

Twenty-five years later—thanks largely to Justice Krever and his exhaustive public inquiry—much has changed, but this story should still serve as a stark reminder that citizens deserve better from the people who are paid to serve and protect the patients in our health care system.

The American Cancer Society projects 178,560 new U.S. cases of melanoma in 2018, more than triple the number of new HIV/AIDS cases. Most teens and health teachers are unaware of the seriousness of melanoma, which is often lethal if not detected early. Melanoma is the most prevalent cancer in the 25-to-29-year age group and second to breast cancer in the 20-to-39-year age group.

In a 2017 survey among 334 teachers, 49 students reported finding early melanomas because of the lessons.

"On 30 May 2018 I wrote to the Minister of Child Safety in Queensland advising her that:

“I am one of many people who sheltered and protected them [the children], in the four years that they were free of ongoing abuse. At various times I drove vast distances to transport them between places of safety… This was one of the greatest privileges of my life to be able to help these children escape the horrific abuse inflicted upon them by fiends, and enabled by Rogue Judges, lawyers and Policemen who actively hid the truth, ignored evidence, and facilitated child rape, effectively trafficking these children to paedophiles.”

"This email was cc’d to The Minister of Police in Queensland, and to Hon Christian Porter, the Federal Attorney General.

"I became involved in this after the late Prof Freda Briggs AO, Australia’s pre-eminent authority of child sexual abuse, asked me as a doctor, to support the mother, using my experience in trying to save other children to help them. The children had been taken from the mother and given to the father, despite multiple disclosures of abuse, so their sexual abuse continued for years.

"…Nearly 40 years of medicine have not provided me with greater grief and horror. My endeavours to protect abused children have resulted in the near complete destruction of my life. These were not my children, yet I could not as a moral man, or as a doctor turn my back on them, and leave them to be abused. I protected them, when the law utterly failed to protect them, because I could do nothing else: I would have been ashamed to do less. I knew I was breaking the law, I also knew that if I did not help them they would be returned for ongoing rape.

"I also knew that it is impossible to evade the law indefinitely, and… despite numerous disclosures by the abused children, to the child protection authorities, the perpetrators of child abuse remain at large, untroubled by the law. Contrast this to the vast AFP operation to apprehend those who risked everything to protect these children.

Intelligent Medical Imaging, Inc. (Florida) has designed the Micro21 microscopy workstation that successfully integrates neural network technology into blood analysis to identify and display white blood cells as well as red blood cells.

ATL Ultrasound, Inc. (Seattle-based firm) has developed a range of diagnostic ultrasound systems for imaging and monitoring cardiac tissue structures and activity. This system utilises an adaptive intelligence algorithm to scrutinise tissues by optimising millions of parameters during a patient examination, thereby eliminating irrelevant frequencies in returned signals.

Neuromedical Systems, Inc. (New Jersey) uses an application of neural networks to scan Pap smears and identify cells for review during cancer screening.

Apps like Babylon in the UK use AI to give medical consultation based on personal medical history and common medical knowledge. Users report their symptoms into the app, which uses speech recognition to compare against a database of illnesses. Babylon then offers a recommended action, taking into account the user’s medical history.

The startup Sense.ly has developed Molly, a digital nurse to help people monitor patient’s condition and follow up with treatments, between doctor visits. The program uses machine learning to support patients, specialising in chronic illnesses.

Developing pharmaceuticals through clinical trials can take more than a decade and cost billions of dollars. Making this process faster and cheaper could change the world. Amidst the recent Ebola virus scare, a program powered by AI was used to scan existing medicines that could be redesigned to fight the disease.

The program found two medications that may reduce Ebola infectivity in one day, when analysis of this type generally takes months or years – a difference that could mean saving thousands of lives.

Some of the key influencing factors like maturity attained by the AI systems, user acceptance, and glitches in
communications along with technical infrastructure continue to stand as barriers to the commercialisation opportunities that currently exist for AI. However, given that ongoing innovations in this domain are moving so fast from academia and theory into reality are proof of the growing momentum in this field.

The video highlights the report's findings which include the fact that 75% of appeals of denied claims ultimately resulted in the member's claim denial being reversed. Even more revealing in the report was the fact that 99% of denied claims are never appealed by the member. The Senior Savings Network, an independent group of insurance agents who specialize in Medicare insurance for seniors across the United States, has released the video in an effort to shine a light on the OIG report, which was released without fanfare on the Health and Human Services website. Christopher L. In the video, the quote from the OIG report that is perhaps most troubling points to the possible motive of such routine denials, "A central concern about the capitated model used in Medicare Advantage (also known as Medicare Part C) is the potential incentive for insurers to inappropriately deny access to services and payment in an attempt to increase their profits."

Medicare Advantage has been an increasingly popular alternative to original Medicare for seniors turning 65 and making the choice between Medicare-related coverage. Medicare Advantage plans are paid a monthly payment by Medicare in order to manage the care for its senior members, thus relieving Medicare of many variable risks associated with the fee-for-service model of the original Medicare. The alternative to this form of managed care involves having original Medicare along with a Medicare Supplement plan which pays most of the costs that original Medicare does not pay.

While the Senior Savings Network assists seniors considering their Medicare options and provides both Medicare Advantage and original Medicare with Medicare Supplement, it is critical that seniors considering their options also factor in their access to care. Seniors considering Medicare Advantage already understand that, in most cases, they must utilize a contracted network of providers, pay co-payments and other costs, but seldom understand the pre-approval and "managed" component to their care. As the OIG report stated, "Under managed care, MAO's (Advantage plans) need to balance managing healthcare costs and utilization while ensuring beneficiary access to quality care."

SANTA MONICA, CA, UNITED STATES, October 31, 2018 /EINPresswire.com/ — In 2011, The Department of Health and Human Services’ Office for Civil Rights (OCR) began auditing healthcare providers and business associates to determine overall compliance with HIPAA’s privacy and security laws. At a recent HIPAA security conference, OCR Director Roger Severino announced that the next round of examinations will be focused on enforcement and the upcoming audits will use harsher investigative tools to hold bad actors accountable.

Enforcement for noncompliant offenders may include subpoenas, legal action, reimbursements to victims, penalties, and more. Additionally, Bloomberg Law recently reported that OCR has been ratcheting up enforcement actions over the past three years, and as random HIPAA audits occur, increased penalties will most likely result.

Jeff Broudy, CEO of PCIHIPAA states, "Overall we see less than 20% of all practices and business associates have implemented the safeguards required under HIPAA. Under the HIPAA Notification Rule, covered entities that experience a HIPAA data breach must self-report the breach to HHS. Some practices aren’t aware of the rules, so audits will help with compliance and overall enforcement. Penalties are no longer immaterial. Average fines range from $100 to $50,000 per HIPAA violation, and are capped at $1.5 million per year.

HIPAA compliance must be addressed continuously. It’s not a checkbox or a “one and done” process. Also, the same HIPAA safeguards required by a hospital or a health plan also apply to dentists, doctors, and their business associates. Anthem’s recent $16 million dollar HIPAA fine, and Mr. Severino’s position above, should be a warning to all healthcare providers and business associates.

OCR’s recent audit results show a lack of compliance throughout the industry. Recurring non-compliance issues include:

¥ Lack of execution of Business Associate Agreements
¥ No HIPAA security risk assessment on file
¥ A failure to manage identified risks
¥ Lack of transmission security
¥ Lack of appropriate internal auditing
¥ No patching of software
¥ Insider threats
¥ Improper disposal of Protected Health Information (PHI); and
¥ Insufficient data backup and contingency planning

Common risk mitigation and corrective action plans that covered entities and business associates may be required to incorporate for compliance include:

¥ Updating risk analysis and risk management plans
¥ Updating policies and procedures
¥ Training of workforce members
¥ Implementing specific technical or other safeguards
¥ Mitigating common risks like utilizing encryption solutions
¥ Improved employee and system monitoring
